Dear Parent(s),

The last three pages of a packet that you will be receiving (April) contain registration forms that need to be completed by you (and possibly your doctor).

1.      Nature's Classroom Student Registration Form/Medical Permission

2.      Nature's Classroom Home & Health Questionnaire

3.      Nature's Classroom Medication Administration Form

4.      The South Colonie Central Schools, “Field Trip Permission, Emergency and Medical information” form. This form is for our own school nurse to use and reference while with the students that week.

Please fill in the necessary information on sheets, tear off the registration forms, and return them to your child’s homeroom teacher. 

If your child may/will require medication while at Nature's Classroom, it is necessary to have a written order from your doctor, physician assistant, or nurse practitioner, to ensure that these medications are safely and legally dispensed.  (Orders are also required for over the counter medications such as Tylenol/Motrin/Dramamine/Pepto Bismol, etc.)  If your child already has an order for medication in school, you do not need a new order.  Please note “Order on File” on the medication form if this is the case.

By Monday May 3, any medication your child may need at Nature’s Classroom must be turned in to the Health Office.  All medication must be in the original container, and accompanied by the order.   It should be placed in a Ziploc bag, labeled with the student’s name, section, teacher’s name

Important Memo regarding Nature’s Classroom

 

Parents/Guardians,

         

          It is important for you to know that you must get a written order from a physician if you want your child to take ANY medication during the field trip.

 

          Unfortunately, there is a form that has a “MEDICAL PERMISSION SLIP” and a statement that gives Nature’s Classroom staff permission to give your child IBUPROFEN  or TYLENOL.  This form is not applicable to the South Colonie CSD nurse, Mrs. Rowland, who will be dispensing medication to your child. 

 

          If you signed the Nature’s Classroom form, Mrs. Rowland will NOT be able to give your child any medication.  You must have the South Colonie “FIELD TRIP PERMISSION, EMERGENCY AND MEDICAL INFORMATION” form completed (front and back) with medication orders written and signed by a doctor, if you want your child to receive ANY medication.
